Marriage & Civil Union Registration Monday to Friday appointments recommended but not necessary

  • Application and information can be found http://nj.gov/health/vital any questions please call the registrar office at 856-678-3089 x 143. Applicant can be picked up in this office.
  • Proof of residency in Pennsville: photo drivers license with current address. If address is different or you do not have a photo ID you must also bring two alternate forms of ID:

Utility Bill / Vehicle Registration / Insurance Card / Bank Statement

  • Identification: Birth Certificate
  • Application fee: $28.00 Check or Money Order payable to the Township of Pennsville
  • One witness who knows both parties, is 18 or older, must be present when applying
  • Previous Domestic Partnership/Marriage/Civil Union that was dissolved legally? You must bring a copy of the final court judgment. In the case of a death, bring the death certificate.
  • There is a three day / 72 hours waiting period
  • You are responsible for arrangements with a Religious officiate, county clerk, county surrogate or superior court judge.
  • You must provide the date, place and name of person performing the ceremony.
  • Print, fill-in, and bring Application with you.
  • The Application is good for six months in this office. Upon issuance, the license remains valid for 30 days!

Domestic Partnership

  • Partnerships are not considered registered until the Affidavit has been filed with a Registrar’s Office and the Certificate of Domestic Partnership has been issued.
  • Couples must be same sex couples OR opposite sex couples who are both 63 years of age or older.
  • Applicants must obtain an Affidavit of Domestic Partnership. The Affidavit must be completed and signed by both applicants at the same time, in the presence of a notary public.
  • The Affidavit must then be filed with the Registrar of Vital Statistics and the required fee of $28.00 check or money order payable to the Township of Pennsville.
  • Print, fill-in, and bring Application with you.
  • Appointments are required
  • No application will be accepted after 3:00 pm.

  • Who can request copies of vital records?
    1. Documents are only issued to persons who have a direct link to the individual(s) named on the vital record event, as identified in Governor McGreevey’s Executive Order 18, and provided that the requestor is able to identify the vital record and can provide proof of his identify and relationship.
    2. If you are requesting a vital statistic record for someone other than yourself, you must be an immediate member of the family. This includes parents, legal guardians, spouses, children, grandchildren, and siblings of legal age.  You must establish relationship. Please note that a grandparent, aunt, uncle, mother-in-law, father-in-law, son-in-law, or daughter-in-law cannot request a vital statistic record.
  • Requesting documents in person
    1. Bring Application to Request a Vital Record
    2. Documents identifying requestor. Acceptable documents are a photo drivers license; a non-photo drivers license and an alternate ID showing the same address; a photo ID and an alternate ID showing the same address; or two alternate IDs
    3. Alternate IDs include vehicle registration, insurance card, voter registration, passport, green card, County ID, school ID, or utility bill
    4. Fee: $25 initial certified copy $2.00 for additional copies.
    5. Payment can be made by check or money order. NO credit or debit cards. Payable to the Township of Pennsville
    6. No transactions after 3:00 p.m.
  • Requesting documents by mail
    1. Follow “in person” instructions above, but do not send original documents. Copies only.
    2. Do not send cash through the mail. Payment by check or money order.  Payable to the Township of Pennsville.

  • Special Information about Death Certificates
    1. Death certificates may also be requested by:
      → Executor of the Estate of the Deceased. (Please note that you will need to provide copies of the Executor papers.)→ Attorney or legal representative for an estate. (Please note the attorney must provide 1) A copy of the attorney’s photo identification; 2) Proof that he/she is the attorney for the estate of the deceased 3) Specific requested record information including name, date of death, place of death, parents’ names OR social security number of the deceased.→ Funeral Director on Record→ Deceased Person’s primary caregiver

       

      → Deceased Person’s legal guardian

      → The Informant listed on the Death Certificate

      → Anyone else possessing a court order allowing them to obtain a specific death certificate
